To save time, we encourage you to print and complete the registration forms provided below before your visit.

If you need a referral, you can also print the provider referral form and take it to the provider of your choice, who can send the necessary referral directly to us.

Referrals:

All new patients require a referral from their physician to begin our new patient process. The referring physician will need to fax the completed referral form along with the patient’s medical records to our office. We will gather any additional necessary records from other providers as needed. Once we have all the information, our physicians will review the patient’s case to determine whether they feel they can provide effective treatment. If approved, our friendly office staff will promptly reach out to schedule an appointment.

We understand that dealing with pain can be difficult, and we strive to complete this process as quickly as possible. If immediate treatment is necessary, we will do our best to accommodate you.

What to Expect on Your Procedure Day

When you arrive for your procedure, you will be greeted by our friendly staff and checked in. A member of our nursing team will then call you back for a brief evaluation to ensure it’s appropriate to proceed with the scheduled procedure. During this time, they will review to make sure you have stopped any necessary medications, like blood thinners, and that you are not currently ill or taking antibiotics.

To ensure your procedure is safe and successful, we want to limit the risk of any complications. For this reason, we strongly advise following the specific protocols our physicians have outlined for blood thinners and antibiotics. These protocols are in place to help everything go smoothly and reduce the risk of complications. Please review the Guidelines for Blood Thinners and Antibiotics before your procedure to ensure your safety and minimize potential risks.

After this evaluation, you will be asked to sign a consent form and will have another opportunity to discuss the risks and benefits with both the staff and your provider.

If sedation is requested, the nurse will administer it prior to the procedure and continue to monitor you throughout. For patients receiving sedation, a chaperone must be present on-site during the entire visit. In most cases, a driver will be required for your procedure, though there are some exceptions. Please ask our office staff or your provider if this requirement applies to you.

Once everything is in order, your procedure will be performed, and you will be monitored for an appropriate amount of time afterward to ensure a smooth recovery.

We aim to make your procedure day as comfortable and seamless as possible, and we are here to assist with any questions or concerns you may have.
